A recent study published in Radiology on July 16 revealed that multiparametric MRI ( mpMRI)-guided targeted biopsy may be more effective in detecting aggressive Prostate cancer subtypes compared to systematic biopsy . The research , led by Sangeet Ghai , MD , from the University of Toronto in Canada , focused on patients suspected of having prostate cancer . The group that received mpMRI - based targeted biopsy showed a higher detection rate of the cribriform ( Cr ) and intraductal carcinoma ( IDC ) patterns , which are indicators of aggressiveness and are associated with poor clinical outcomes.

The detection of these subtypes on MRI has been a topic of debate , and the researchers aimed to compare the effectiveness of mpMRI - targeted biopsy with systematic biopsy in detecting these patterns . The study included 453 participants , with 226 in the systematic biopsy group and 227 in the mpMRI - targeted biopsy group . Patients with a Prostate Imaging Reporting and Data System ( PI - RADS ) score of at least three in the mpMRI group received a targeted biopsy.

The results showed that 25 % of the biopsies in the mpMRI group identified prostate cancer with the CR or IDC subtypes , while only 16 % of the biopsies in the systematic biopsy group did so . This difference was statistically significant . Additionally , patients with clinically significant prostate cancer and positive Cr / IDC patterns had a higher mean cancer core length compared to those with negative patterns . The researchers concluded that more studies are needed to determine if obtaining more biopsy samples from MRI targets can improve the detection of these subtypes.

In an editorial accompanying the study , Michele Scialpi , MD , from the University of Perugia , and Eugenio Martorana , MD , from the New Civil Hospital of Sassuolo in Italy , emphasized the importance of further research to establish the role of MRI and biopsy in diagnosing the Cr / IDC pattern . While MRI - targeted biopsy showed promise in increasing the detection of these patterns , more evidence is needed to confirm its reliability.
